HealthWorks offers low cost health clinic offering in-house labs, physicians specializing in family practice and internal medicine, on-site pharmacy, diabetic clinic, dental clinic and social work/case management.

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.

The Wyoming Home Services (WyHS) program is used to provide services to those qualified individuals who are at risk of premature institutionalization.
