We do not charge any out of pocket fee's. What many other agencies charge as private pay case management we see as an opportunity to give back to our community.

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.
