Alzheimer's Association delivers education & awareness presentations that promote risk reduction, early detection, and diagnosis of Alzheimer's disease and all other dementia. Case management is a required service on all Waivers to assist participants in gaining access to needed Waiver and other Medicaid State Plan services, as well as medical, social, educational, community supports and other support services, regardless of the funding source.

Veteran callers are connected to local Service Officers that can inform veterans & family members about state and federal benefits. Service Officers assist with forms, claims and coordination, the Medals for Veterans Program, and the Incarcerated Veterans Program. Outreach is conducted throughout the state and territories.

Hands 2 Help has been providing compassionate home health care services since 1992. We provide caregiver services that supplement the restricted services covered by health insurance companies. Clients do not have to be home bound and may request services on a regular basis such as every weekday at a certain time or whenever help is needed such as when a regular caregiver or family member is temporarily unavailable. Care is also available on a one-time-only basis such as before or after surgery.

Lander Medical Clinic and Western Family Care together are a locally owned, multispecialty group practice. Our clinicians provide care in Behavioral Health, Family Medicine, Gynecology, Internal Medicine, Pediatrics, Dietetics, and Same-day Care in a caring and trusted environment. Other services include an onsite lab, infusion, ultrasound, echos & stress tests and mammography.

CWCC provides mental health services to clients including family and group counseling, psychiatric services, individual, family and marital therapy and evaluations/assessments.
