The Family Medical Center (FMC) is Johnson County Healthcare Center’s outpatient medical clinic.

Parents and their loved ones who contact the Hotline will speak to professional counselors. Counselors will immediately provide real-time support, information, and resources.

The Community Health Center Pharmacy assists thousands of patients every year. Our pharmacists are on hand to not only fill your prescription requests but also to discuss medication side effects and medication interactions. We also work with our Fremont County clinics and patients to offer the same pharmaceutical services. To help save you time and money, we will ship your prescriptions to your local clinic.

We provide a wide range of services designed to meet the diverse needs of our clients. From everyday assistance to specialized care, our team is here to bring comfort, dignity, and peace of mind, especially for rural communities and our honored veterans in Colorado and Wyoming.

Our property offers one bedroom apartments for persons who are 62 years and older and the mobility impaired. Our units are all HUD subsidized. The rent is based on 30% of adjusted income. We offer free internet service, weekend meal delivery service, and a Service Coordinator that assist our residents in finding the services they need to remain living independently.

Better Dementia, led by Amy Shaw, PA, provides compassionate education and one-on-one consulting for families caring for someone with dementia. Amy helps families understand what to expect, how to respond, and how to create calm, confident care at every stage of the journey.

The Wyoming Home Services (WyHS) program is used to provide services to those qualified individuals who are at risk of premature institutionalization. These services are designed to keep people in the least restrictive environment for as long as possible. The Shoshoni Senior Center offers meals and services for senior citizens. Those age 60 and older can register, but all ages are welcome to join us as a guest for meals and activities. We offer activities, exercise equipment, a lending library, coffee, public-use computers, and other services. The Senior Center is a community gathering place and also offers visitor information.
