Our Community Living Center (CLC) unit provides hospice, long term and acute rehab care. The Old Glory CLC is a long-term care unit for veterans who meet eligibility conditions. Eligibility is based on service connection/employability for long-term care and respite care. The ECU is located on the first floor of the main building, in the east end.

While at The Life House, clients are involved in various therapeutic, recreational and community, recovery-oriented activities. Each client is assigned a Clinician upon admission and given a thorough intake interview following which an individualized treatment plan is developed.

Others, including teachers and babysitters, are entrusted with precious young lives that could require aid on a moment’s notice.

Memorial Hospital of Carbon County participates in the State of Wyoming’s Department of Health, Aging Division, Community Living sections, Wyoming Home Services program. The program provides services to Wyoming Senior Citizens and disabled adults ages eighteen (18) years of age or older who experience difficulty performing daily tasks on their own. Priorities shall be given to persons who are at risk for premature institutionalization and be in need of the program.
