At Cody Regional Health’s Cedar Mountain Center (CMC), we believe no one is incapable of overcoming addiction and chemical dependency. Crisis services are tailored to each individual and often include a crisis shelter stay for youth. Intake plans are driven by each client and their family to identify and address crisis and long-term needs. Reasons for intake include; emergency care and shelter, respite or therapeutic breaks, and group home services.

Those Veterans in need of services not available at a particular VA clinic are either referred to the Sheridan VA Medical Center, another VA facility within the region, or are scheduled locally for fee basis care within the local community. For emergency care, all Sheridan VA clinics activate the local 911 system and veterans are taken to the local emergency medical facility.

Fort Collins VA Clinic

We provide services to all individuals in their homes, assisted living facilities, nursing facilities, homeless shelter, independent living facilities and any other community agency. We provide assistance with Medicare, Medicaid, SS, Disability, Insurance, advocacy, support, mental health treatment, coordination of care, Medicaid Waiver case management, transportation, check-in visits, mental health assessments, capacity, and more.

We are staffed with family physicians, pediatricians, physician assistant, family nurse practitioners, psychologists, dentists, dietician, public health nurses and optometrists. Specialty clinics are also held at our facilities ranging from orthopedic, podiatry, nephrology, obstetrics, audiology, women's health and cardiopulmonary services.

Cody Regional Health Home Health provides care for patients in the home setting within a defined service area, including Cody, Powell, Meeteetse, Burlington and Ralston. Home health services are for patients who need attentive care and have trouble leaving home on their own. Nurses are on call 24 hours a day, 7 days per week. You or your loved one may receive care from a team of home health professionals who work together to provide comprehensive, well-rounded care.
