Community EMS is a patient-centered model using mobile resources to deliver care outside traditional settings, aiming to improve outcomes and reduce ER visits. The program will refer patients to Laramie County resources, conduct home visits, and assess safety. Initial efforts focus on voluntary home safety inspections to prevent falls, reconcile medications, and install or replace smoke and carbon monoxide detectors.
