ACE CASE MANAGEMENT
Provides case management services under the Community Choices Waiver (CCW) program which serves individuals who are aged 65 and older, or individuals who are aged 19-64 who have a disability that is verified as meeting Social Security Administration (SSA) disability determination criteria.
