Eligibility
Must reside in the city of Laramie or Cheyenne or its environs. Must have a primary care physician who certifies the delivery of cares. If through Medicaid, then you must be receiving case management from a different agency.

Fees
For Private Pay only: $125.00 one time admission fee, $25.00 an hour for Personal Support Services, $35.00 an hour for Home Health Aide services, $75.00 an hour for Med Management by an RN, $110.00 for all other Skilled Nursing cares.
